Why families choose us

Good coordination should make the NDIS feel clearer over time

A Support Coordinator may be working across therapy, daily supports, equipment, housing, health services, family input and community connections at the same time. Multibility Care helps participants see how those moving parts relate to one another, identify where action is required and follow up when progress stalls. We also keep capacity building in view so participants can become more confident communicating with providers, comparing options and making decisions about their own support network.

We explain what matters without adding more jargon

Funding categories, service agreements and provider responsibilities can become confusing quickly. We work through the plan in practical language, clarify which actions may relate to current goals and help identify useful questions before new services begin. Better information gives participants more room to make informed decisions instead of feeling rushed into the first available option.

Your choice stays visible when several services are involved

A large support network can create pressure from many directions. Coordination should not replace the participant's decision-making. Our role is to help organise information, compare options, follow up providers and make the next step clearer so participants can remain involved in decisions about the services they use and the outcomes they are working towards.

Local & area support

Support Coordination across the Sunshine Coast

Support Coordination needs to work in the real world, not only inside an NDIS plan. On the Sunshine Coast, participants may have providers spread between major coastal centres, residential suburbs and hinterland communities. Travel time, availability, appointment locations and the range of local services can all affect how practical a support network feels from week to week.

We can help explore options around Caloundra, Kawana, Maroochydore, Mooloolaba, Noosa, Nambour and surrounding communities, while also considering mainstream and community supports where they may complement funded services. Familiar local places such as Sunshine Plaza, Caloundra markets, the Mooloolaba foreshore and Noosa National Park may also form part of broader participation and community goals.

Local knowledge can reduce false starts. Understanding provider locations, travel time, availability and local service patterns can help make coordination more practical from the beginning.

Coordination for the parts of your plan that need practical attention

Support Coordination is often about keeping many smaller actions moving at the same time. One provider may need a referral, another may be waiting for information, a review date may be approaching and a service may no longer suit the participant. We help organise those priorities into a clearer sequence while keeping choice, capacity building and long-term goals visible.

  • 01

    Plan Understanding

We help break the NDIS plan into practical parts so participants can see what funding is available, what it may relate to and which priorities require action first.

    • Review funded supports and current plan priorities
    • Explain service options and responsibilities in plain language
    • Identify useful actions for the current plan period
  • 02

    Provider Connections

Finding a provider is about more than locating a service name. Availability, communication, experience, travel, waiting lists and how well the provider fits the participant all matter.

    • Identify suitable local and broader provider options
    • Assist with referrals, initial contact and follow-up
    • Compare practical options before services are confirmed
  • 03

    Capacity Building

Coordination should help participants become more confident making choices, communicating with providers and managing more aspects of their own support network where that is realistic.

    • Build confidence asking providers questions
    • Develop practical service management and decision-making skills
    • Understand agreements, responsibilities and available choices
  • 04

    Crisis & Complex Situations

When arrangements break down or circumstances change quickly, coordination can help clarify immediate priorities and create a more organised pathway for the next steps.

    • Clarify urgent gaps in existing support arrangements
    • Coordinate communication between relevant services
    • Identify practical short-term options while longer plans develop
  • 05

    Plan Review Preparation

A review is easier to prepare for when goals, provider information, reports and unresolved support needs are considered before important deadlines arrive.

    • Review goals and how current supports are working
    • Identify changes, service gaps and unresolved needs
    • Help organise relevant reports and supporting information
  • 06

    Mainstream & Community Links

Not every useful service sits inside NDIS funding. Coordination can also help participants identify health, mainstream and community resources that complement funded supports.

    • Explore health, education and mainstream service connections
    • Identify suitable community groups and local resources
    • Build a broader network around individual goals and daily life
How coordination gets moving

A clear sequence from understanding the plan to preparing for what comes next

01
Meet & Review

We look at the current plan, participant goals, existing providers, service gaps, upcoming dates and anything already causing difficulty.

02
Connect

We identify suitable services, make referrals, follow up provider availability and help compare options before new arrangements are confirmed.

03
Build Skills

Participants are supported to understand agreements, communicate with providers and manage more plan decisions where that suits their goals.

04
Review & Plan

We keep track of what is working, what has changed and what information should be ready before the next plan discussion or review.

Building coordination confidence

Good coordination should help you understand more over time

For many participants, one of the long-term benefits of Support Coordination is becoming more confident asking questions, comparing providers and understanding how services connect with the plan. Some people may continue to need coordination because their situation is complex, while others may gradually take on more responsibility themselves. The pace should reflect the participant's circumstances, preferences and goals.

Coordination keeps the focus on your decisions
  • You remain involved in choosing providers and support arrangements.
  • Information is explained so options can be compared more confidently.
  • Skills can build gradually as plan management becomes more familiar.
  • Support can adapt when goals, providers or circumstances change.
Questions answered

Frequently Asked Questions

Practical answers for participants, families and referrers considering Support Coordination with Multibility Care on the Sunshine Coast.

Support Coordination is an NDIS-funded capacity building support included in some participant plans. A Support Coordinator can help you understand your plan, connect with suitable providers, organise referrals, work through service issues and prepare for reviews. The role is also intended to build confidence so participants can become more involved in directing and managing their own support arrangements where possible.

Your plan should indicate whether funding has been included for Support Coordination or another level of coordination support. The wording can sometimes be difficult to interpret, so if you are unsure, contact our team and explain what appears in your plan. We can help identify the information that needs to be checked before coordination services begin.

Our Sunshine Coast service area includes Caloundra, Maroochydore, Mooloolaba, Buderim, Nambour, Kawana, Noosa, Noosaville, Coolum, Peregian, Mountain Creek, Sippy Downs, Currimundi, Wurtulla, Warana, Bli Bli, Yandina, Eumundi, Maleny and Mapleton. If you are close to these communities but do not see your suburb listed, contact our team so we can confirm current arrangements for your location.

Participants can contact Multibility Care directly, or an appropriate family member, nominee, plan manager, allied health professional or other person involved in the participant's support can submit a referral. We will need enough information to understand the participant's location, plan, coordination funding, current providers, immediate priorities and any important upcoming dates.

Start times depend on current coordinator availability, participant location, the complexity of the request and whether there are urgent matters that need to be understood before services begin. Once the essential information is available, we can explain the practical steps required before the first coordination session and consider important deadlines such as upcoming reviews or provider changes.

Support Coordination generally focuses on understanding the plan, connecting services, building participant capacity and helping different providers work together. Specialist Support Coordination is designed for situations with a higher level of complexity where significant barriers, risks or service system issues may affect the participant's ability to access and maintain supports. The participant's plan and circumstances influence which level of coordination is appropriate.

Participants have choice over the providers they use. If you are considering changing coordinators, it is useful to review any notice requirements in your current service agreement and identify important information that should transfer. We can discuss existing providers, upcoming deadlines, incomplete referrals and immediate priorities so a transition can be planned with as little disruption as practical.
