Why families choose us

Community support should help people belong, not simply attend

Being physically present in the community is only one part of meaningful participation. A participant may want to reconnect with friends, become more confident using local services, attend appointments with less stress, volunteer, explore hobbies or gradually become comfortable in places they have avoided before. Multibility Care takes time to understand those individual goals so community access support can feel purposeful and connected to the participant's real life.

Community Access Support for participants across the Fraser CoastSupport can begin with familiar places and routines, then develop as confidence, independence and community connection grow.

Built around interests, routines and real goals

Community access should reflect what the participant actually wants from their week. That could include visiting the Hervey Bay esplanade, going shopping, attending an appointment, joining a group, exploring volunteering or simply developing a comfortable routine around getting out of the house. We focus on what is meaningful rather than filling time with generic activities.

Familiar support can make new experiences easier

New places and changing routines can sometimes feel overwhelming. Consistent support can help participants prepare, understand what to expect and practise the same community routines over time. That familiarity can reduce stress and create opportunities for greater independence without pushing someone faster than feels comfortable.

Community Access Support across the Fraser Coast

The Fraser Coast offers a wide mix of everyday community environments, from Hervey Bay and Maryborough through to coastal townships, smaller communities and surrounding regional areas. Support can be planned around local routines such as appointments, shops, social activities, recreation, family contact and community events.

Participants may already have favourite places around the Hervey Bay esplanade, Mary River, Burrum Heads or the K'gari ferry terminals, while others may be looking to discover new activities closer to home. Our role is to make participation more manageable and meaningful, with support that fits the participant's goals, communication needs and confidence level.

Community Access Support should connect with the life a participant actually wants to live. Some people want more social connection. Your pathway

From an idea to confident community participation

01
Discover Your Interests

We talk about the places you enjoy, activities you want to try, existing routines and any barriers that currently make participation more difficult.

02
Plan Your Outings

We consider locations, timing, transport and the level of assistance that will help each activity feel comfortable and worthwhile.

03
Meet Your Worker

We consider communication style, experience, shared interests and the type of working relationship that helps the participant feel understood.

04
Get Out & Grow

As community routines become more familiar, support can evolve with the participant and create new opportunities for confidence and independence.

What progress can look like

Everyday experiences can become meaningful milestones

Community progress is often found in small changes. It might be ordering independently at a cafe, staying longer at an activity, learning the route to a regular appointment, speaking with someone new or making more choices during an outing. Those moments can build confidence because they happen in the places where everyday life actually takes place.

Support keeps the focus on you
  • Activities are shaped around your interests and goals.
  • Confidence can develop gradually through familiar routines.
  • Support can adjust as skills and independence grow.
  • Participation should feel meaningful rather than simply fill time.
Questions answered

Frequently Asked Questions

Community access can involve transport, social activities, appointments, skill building and new experiences. These answers cover common questions from participants, families and referrers looking for Community Access Support across the Fraser Coast.

Community Access Support is assistance that helps participants take part in activities and everyday life outside the home. Depending on individual goals and funding, it may include social participation, outings, transport support, appointments, volunteering, events and skill building in real community settings. The purpose is to reduce barriers to participation while supporting greater confidence and independence.

Whether a particular community access service can be funded depends on the supports included in your NDIS plan, your goals and how the requested activity relates to your disability support needs. When you contact Multibility Care, tell us what type of community assistance you are looking for and how your plan is managed so we can discuss the service options with you.

Our Fraser Coast service area includes Hervey Bay, Maryborough, Urangan, Pialba, Scarness, Torquay, Toogoom, Burrum Heads, River Heads, Craignish, Kawungan, Wondunna, Tiaro, Howard, Childers, Booral, Takura, Bundaberg, Aldershot and Bealka Creek. If you live close to these communities but do not see your location listed, contact our team so we can discuss your area.

You can contact Multibility Care directly or complete our online NDIS referral. Referrals may come from participants, family members, nominees, support coordinators or other appropriate people involved in the participant's support. We will ask about goals, preferred activities, location, support needs and relevant NDIS details before discussing the next steps.

Start times vary depending on the participant's location, goals, support requirements, NDIS funding information and suitable worker availability. Some arrangements are straightforward while others need more planning around transport, communication needs or particular activities. Once we understand what you need, we can explain the practical steps for getting support underway.

Community participation can be delivered in different ways depending on the participant's support needs, preferences, goals and available funding. Some people benefit from one-to-one support, while others may enjoy suitable group activities or shared experiences. We discuss what type of arrangement is appropriate rather than assuming everyone wants to participate in the same way.

It can. Community settings give participants opportunities to practise travel routines, communication, decision-making, planning and other everyday skills. Independence may develop gradually, for example by needing fewer prompts in a familiar environment or taking more responsibility for part of an outing. Support should respond to those changes while remaining available where assistance is still needed.
