Support Coordination - Gympie

Support Coordination Gympie Understand the plan, organise the moving parts and know what needs to happen next.

Multibility Care provides Support Coordination across the Gympie region for NDIS participants who want clearer direction around their plan, providers and next steps. A coordinator can help make sense of funding, follow up actions, connect services, prepare for reviews and work through situations where several providers or systems are involved. The aim is not to make every decision for the participant. It is to make the information easier to use so the participant can make informed choices and build confidence managing more of the plan over time.

  • Clear explanations of funding, services and provider options
  • Help following up actions across the plan
  • Support to build confidence making NDIS decisions
Support Coordination Gympie with Multibility Care
Your plan should be usable, not just readable. We can help turn goals, budgets, referrals and provider conversations into practical actions you can follow.
Support Coordination in Gympie with Multibility Care
A coordinator can hold the thread when several services are involved. Provider conversations, assessments, referrals and review preparation are easier to track when the next action is clear.
Why families choose us

Good coordination should make the NDIS easier to navigate, not create another layer of confusion

Support Coordination often sits between several parts of a participant's life. There may be allied health professionals, support workers, mainstream services, family members and NDIS providers all contributing different information. Our role is to help organise that information, identify what needs action and keep the participant involved in decisions rather than allowing the process to become something that happens around them.

We explain what the next step is and why it matters.

If an assessment, provider conversation or review preparation task needs attention, we break it into practical actions. Participants and families should know what is being followed up, who is responsible and what information still needs to be gathered.

Building capacity includes knowing when you can manage something yourself.

A coordinator does not need to remain the person who makes every phone call or organises every provider forever. Where appropriate, we can help participants practise those tasks and become more confident managing parts of their plan independently.

Local & area support

Support Coordination across the Gympie region

Multibility Care supports participants across Gympie, South Gympie, Jones Hill and surrounding communities. Local context can matter when a plan involves providers spread across several areas, transport between appointments or connections with mainstream and community services outside the centre of Gympie.

A participant may see an allied health professional in Gympie, use community services near the Mary River and need to connect with providers closer to Pomona, Cooran or the Noosa hinterland. A coordinator can help keep those different services connected to the same goals and identify gaps before they become bigger problems.

Changing coordinators does not mean losing all the work already completed. We can review current providers, outstanding referrals, assessments, service agreements and upcoming review needs so useful information can carry into the new coordination arrangement.

Coordination for the decisions, connections and follow-up behind an NDIS plan

Support Coordination can cover much more than finding a provider. The work often involves understanding what funding is available, comparing services, following up referrals, preparing evidence for reviews and helping the participant respond when something changes unexpectedly. These six areas reflect common parts of coordination work.

Plan Understanding

We help translate plan information into practical decisions so participants understand what funding may be available and what actions need attention.

  • Review goals and funded supports
  • Clarify service priorities
  • Identify actions and unanswered questions

Provider Connections

A coordinator can help identify providers, discuss available options and support the participant to compare services before making a decision.

  • Identify relevant providers
  • Support referrals and introductions
  • Compare options and service fit

Capacity Building

Coordination can include practising plan-management skills so the participant becomes more confident organising services and making decisions.

  • Build confidence contacting providers
  • Practise decision-making skills
  • Increase independence with plan tasks

Crisis & Complex Situations

When services break down or several issues happen at once, coordination can help organise communication and identify the most urgent actions.

  • Prioritise immediate coordination tasks
  • Bring relevant providers into communication
  • Rebuild workable service arrangements

Plan Review Preparation

Review preparation can include checking progress, gathering relevant information and identifying changes that should be clearly explained.

  • Review progress against goals
  • Organise reports and supporting information
  • Prepare questions and priorities for review

Mainstream & Community Links

Not every useful service sits inside the NDIS. Coordination can also help connect participants with community, health and mainstream services.

  • Identify community-based options
  • Connect with relevant mainstream services
  • Reduce unnecessary duplication between systems
How coordination begins

A practical sequence for turning the plan into clear next steps

  1. Meet & Review We look at your current plan, goals, existing providers, funding, outstanding actions and anything that is currently difficult to organise.
  2. Connect We help identify appropriate providers, organise referrals and connect mainstream or community services where they are relevant to your goals.
  3. Build Skills Where appropriate, we help you practise provider communication, decision-making and other coordination tasks you want to manage more independently.
  4. Review & Plan We keep track of progress, identify gaps and prepare for plan reviews or service changes with clearer information about what has and has not worked.

A plan can look organised on paper while the real-life pieces are still disconnected

One provider may be waiting for an assessment. Another may need information from the participant. A referral might have been submitted but not followed up. Funding may exist for a support, but nobody has yet worked out which provider is suitable. Support Coordination helps keep those loose ends visible.

Clear coordination does not mean adding more meetings or paperwork for the sake of it. The useful work is identifying what needs action, making sure the right person has the right information and helping the participant understand the decisions in front of them.

When several systems overlap

The NDIS is only one part of the support picture

Health, housing, community organisations, family supports and other mainstream services may all have a role. Coordination can help make those connections clearer.

  • Keep provider responsibilities visible
  • Follow up referrals and outstanding actions
  • Connect NDIS and mainstream services where needed
  • Keep the participant involved in decisions
Questions answered

Frequently Asked Questions

Support Coordination can involve funding questions, provider changes and preparation for important NDIS decisions. These answers cover some of the issues participants and families commonly ask about.

Support Coordination helps eligible NDIS participants understand and use their plan, connect with providers and organise the different services involved in reaching their goals. A coordinator may help compare provider options, arrange referrals, follow up actions, prepare for plan reviews and connect NDIS services with relevant mainstream or community supports. An important part of the role is also helping participants build confidence managing more of these tasks themselves where appropriate.
Whether Support Coordination can be used depends on the funding and supports approved in your individual NDIS plan. If your plan includes funding for coordination or you are unsure how the relevant funding is described, we can look at the information with you and discuss the practical next step. We will not assume funding is available without checking what is actually included in the participant's plan.
Our Gympie Support Coordination area includes Gympie, South Gympie, Jones Hill, Two Mile, Chatsworth, Monkland, Cedar Pocket, Pie Creek, Tamaree, Goomboorian, Wolvi, Kin Kin, Pomona, Cooran, Cooroy, Eumundi, Kia Ora, Kandanga, Amamoor and Mothar Mountain. Contact our team with your suburb and current coordination needs so we can discuss availability and how support could be arranged.
Participants can contact Multibility Care directly, and referrals can also come from family members, allied health professionals, plan managers, other providers and people involved in the participant's support network. Useful starting information includes the participant's location, current plan details, coordination funding, existing providers, immediate priorities and any outstanding actions that need attention.
Start times vary depending on coordinator availability, the participant's location, funding and whether there are urgent actions already in progress. Once we understand the situation, we can explain current availability and what information is needed to begin. If several providers or referrals are already involved, having a clear summary of what has happened so far can help the transition move more smoothly.
Support Coordination generally focuses on helping participants understand their plan, connect services, build coordination skills and manage provider relationships. Specialist Support Coordination is a higher level of coordination intended for situations involving more significant complexity or barriers. The type of coordination a participant can use depends on what has been approved in the NDIS plan and the participant's individual circumstances.
Participants can discuss changing coordinators if the current arrangement is no longer suitable, subject to any relevant service agreement terms and plan requirements. A useful transition should include information about current providers, outstanding referrals, upcoming reviews, recent assessments and actions that are still in progress. We can discuss what information would help us understand the current position before a new coordination arrangement begins.
