Yes. Health professionals, support coordinators, family members and participants themselves can all submit a referral through our simple online referral form. Once received, our team makes contact within one business day to begin the conversation, confirm funding and goals, and arrange the first meeting.
After your first conversation, we usually arrange an introductory meeting within a few days. Once we understand your goals and confirm your funding, coordination can often begin within one to two weeks, depending on availability and your preferred schedule.

Some participants choose to keep coordination long-term for complex situations, and that is perfectly fine too - the choice is always yours.
