Preparing an NDIS referral
An NDIS referral is a way to introduce the person, the support being considered and the best contact for the next conversation. Use the guided form above to share the information you are authorised to provide. You do not need to describe every part of a person’s life to explain the initial request.
Before starting an NDIS referral
Check who is making the referral, their connection to the person and who should be contacted about the request. If you are referring someone else, make sure their wishes and the relevant permission to share information are understood. Keep the first description focused on the support request and the practical next step.
Information that helps the conversation
A useful NDIS referral explains the support being explored, the person’s general location and the everyday needs or goals relevant to the enquiry. Include known communication or access preferences where appropriate. If a detail is uncertain, describe what you know rather than guessing or adding information just to make the form look complete.
